structured_lifecycle currently serializes create_inner across the full ACP handshake (up to 60 seconds). One slow adapter stalls every other structured spawn. Narrow synchronization to the state transition/claim that actually requires exclusion while preserving shutdown-versus-create correctness, and add a concurrent-spawn regression test.
