feat: per-host claude settings and machine profile
settings.json carries skipDangerousModePermissionPrompt and the rev hook wiring, so yolo gets its own; other hosts keep the shared one. Machine profile picks which agent-skills machines/<name>.md is imported. Co-Authored-By: Claude Opus 5 <noreply@anthropic.com>
