Render sessions as cards in the web sidebar #107

Replace the single-row sidebar leaf in web/src/catalog.ts with a session card, implementing docs/SIDEBAR.md (#101).

Today the row is one line and the action buttons sit on top of the name — on a claude session the buttons cover the label. The card fixes that by giving actions their own anchored position.

  • Line 1: session name, actions (spawn similar, kill) top-right.
  • Line 2, only when it has content per the contract: activity dot in the state's colour plus the duration from activity_since (#102), the ⚠ blocked-work warning (which moves here off the name row), exited, and an Error's detail string.
  • Cards keep the existing directory-tree grouping, the depth indentation, and the #/attach/:id navigation.

The state word is never rendered — the dot's colour is the state. Keep the tooltips and aria-labels carrying the state in words so the meaning is not colour-only.
