Blocks #143. TUI approval failures are applied unconditionally. While the request is pending, the socket may resolve or supersede the approval; a late transport failure then displays against the new or absent approval, including when the decision actually applied. Carry the submitted approval nonce/generation into failure handling and surface failure only while that nonce remains pending. Add regression coverage.

I’ll carry the submitted approval nonce through the TUI effect and gate failure state changes on that nonce still being pending. Regression tests will cover resolved, superseded, and still-matching late failures. Verification: relevant TUI tests plus strict clippy.
