Blocks #143. Every structured tool patch replaces its complete DOM subtree. Running updates collapse expanded output and destroy keyboard focus on the tool header. Preserve expansion and focus across remounts, or update the existing row in place. Add regression coverage.
Plan: add stable-row UI state capture around tool patch remounts, cover expanded and focus behavior in deterministic DOM tests, then run scoped and full web verification before committing and pushing.
