Miguel PalhasandClaude Opus 4.6 b9f8f02e11 fix(maestro): fix diff double-spacing and add +/- markers ...

Block spans + literal \n text nodes inside <pre> caused empty lines
between every diff line. Drop the text nodes, add unified-diff prefixes.
