Hard-failed titles stay queued after they are satisfied #134

The needs_decision conditions in crates/arr-daemon/src/attention.rs count
hard-failed grabs without checking whether the title still needs anything. An
episode imported later from a different release, or one whose intent was
withdrawn, keeps its two old hard-fails and stays queued until those grab rows
age out.

#126 mirrored the existing movie query deliberately rather than diverging, so
both lanes have it. The queue therefore reports titles that need no decision,
which is the failure mode §9.5 is built to avoid — an operator who learns the
queue contains stale entries stops reading it.

Gate both lanes on the title still being unsatisfied: the episode wanted and not
available, the movie wanted and not available. Cover a title that hard-failed
twice and was then imported from a third release: it must leave the queue.
