Extend docs/STRUCTURED-VIEW.md from its current Claude-only scope to support both Claude Code and Codex through provider-aware ACP adapters. The structured view remains opt-in and the persisted resolved command selects the provider; broader web/TUI visual parity is explicitly outside this milestone.
